Medical Forms & Student Health Requirements

Robert Frost Middle School is committed to supporting the health, safety, and well-being of all students. Parents and guardians are encouraged to review and submit the appropriate medical forms to ensure that students receive necessary health services and support during the school day.

Medical documentation may be required for students who need medication administration, authorization to carry or self-administer medication, specialized health care services, or accommodations related to a medical condition.

Important: Medication may only be administered at school when the required medical authorization and parent/guardian consent are on file, in accordance with LAUSD procedures and applicable California law.
